I also take all out-of network benefits. These are benefits that allow you to be directly reimbursed for my services. If your insurance is not listed above, please contact your insurance company to find out what out-of-network benefits you may have.
Payment Policy
All Copays and Co-insurance are due at the time of service. I take all major credit cards.
Cancellation Policy
If you need to reschedule or cancel your appointment, please let me know as soon as possible. Cancellations with less than 24 hours notice will be billed at the full fee.
Questions to Ask your Insurance Company
– What is my mental health deductible? How much of this has been met?
– How much will I be responsible for after each session?
– How many sessions are allowed per year?
– Do I need pre-approval for coverage?
– Do I have out-of-network benefits for mental health?
